CASE 4
Needle core biopsy report: Both cores showed a fibrous dense reaction with rare chronic inflammatory infiltrates. A well-outlined round structure appears to be filled with carcinomatous pleomorphic cells suggesting a DCIS foci. Serial sections proved these cells were located in lymphatic micro-vessels. The NCB was reported in category B5.
Triple assessment:
- Clinic: residual changes after mastitis?
- Mammography: benign findings.
- Ultrasound: benign findings.
- NCB: malignant, lymphangitis carcinomatosa.
Ccl: In the absence of clinical sign of lymphangitis carcinomatosa, a mastectomy with axillary clearance was decided.
Histology report: Presence of a 1 cm large invasive ductal carcinoma NOS grade II in the lower outer quadrant surrounded by fibrosis and many neoplastic lymphatic emboli. Eight metastasis amongst the 10 sampled lymph nodes.
Further treatment: radio- and chemotherapy.
