CASE 8
FNAC:, Some solid clusters of epithelial cells are found in an hemorragic background. Light overlapping, good cohesion but presence of round nuclei with coarse granular chromatin and nucleoli. The FNAC was classified in category C3.
Needle core biopsy report: Presence of a solid tumor with papillary aspects and some rigid cribriform architecture. The NCB was reported in category B5 and suggests a papillary carcinoma. No definite prove for invasion.
Triple assessment:
- Clinic: small palpable mass.
- Mammography: round tumor. Mucinous or medullary carcinoma are proposed.
- Ultrasound: suspicious.
- FNAC: adequate, representative, atypia probably benign C3
- NCB: adequate and representative, malignant, intracystic (?) papillary carcinoma, B5.
Ccl: Lumpectomy. No axillary resection regarding the age of the patient.
Histology report: Intracystic papillary cribriform carcinoma with DCIS low nuclear grade. No evidence of invasive tumor.
Further treatment: Tamoxifen.
